We will send you a simple test kit in the post.
The test kit includes a pot for a urine sample, some safe packaging and a freepost envelope. All you have to do is pee in the pot and send it back to us.
You'll also find a short form to complete so that we can contact you. We will only use this information to contact you – it stays totally confidential. But please remember to send this back in the envelope with your pot.
